The Hospital@Home program in partnership with North Memorial Health is an example of what happens when you put the needs of people first and build solutions to keep them healthier in the communities where they live. This innovation has opened the door to opportunities which deepen relationships between consumers and their providers and pave the way for the hospital of the future achieving long-term triple aim  – improved customer experience, improved health of our populations, and lower costs.
