Lifespark COMPLETE offers a whole-person approach designed to help you:

• and ultimately live a fuller, more independent life as you age.
Lifespark COMPLETE is a concierge level program that meets your health care needs in the comfort and privacy of your home. A team of senior-savvy Advanced Practice Providers serve as your primary care and work in collaboration with physicians who regularly see you. Plus, you have access to Urgent Response Services to rule in or out whether you should go to the emergency department. This includes telehealth, addressing questions or, when needed, deploying Mobile Urgent Responders who can see you for urgent situations, avoiding trips to the ER.
Lifespark COMPLETE is available to residents with partnering Medicare Advantage plans.

Lifespark offers two exceptional options for onsite provider services: Lifespark Health and Lifespark COMPLETE. If your current health plan is not a partnering plan for Lifespark COMPLETE, you may have a plan that qualifies for Lifespark Health. A Lifespark Enrollment Specialist can assist you in confirming your eligibility, just call 952-873-7686.
During Medicare’s Annual Enrollment Period, October 15 – December 7, members can select or change Medicare plans for the coming year. If you are not eligible under your current plan, you have time to change your plan to become eligible for either Lifespark Health or Lifespark COMPLETE.

Lifespark Health provides the convenience of clinic level care in your home. Lifespark’s team of senior-savvy Advance Practice Providers see clients for scheduled visits and coordinate care with the integrated onsite team.
Lifespark Health is available to residents with Medicare, most Medicare Advantage plans or private pay.
